Individual Online Learning Course

This Enrollment Form (the “Enrollment Form”) is part of the Terms of Use (the “Agreement”)  entered into by and between EMA and Customer. The Enrollment Form is hereby incorporated by reference into and made part of the Agreement and the Enrollment Form shall become effective as of the same date (“Effective Date”).

Specifications of the Individual Online Learning Course

Customer shall be entitled to designate one User who will have access to the use of the professional development course identified on this webpage, which will be made available by EMA to the one User as specified below for one calendar year:

  • The asynchronous course identified on this webpage and made available by EMA through the Thinkific platform (subject to Thinkific’s terms and conditions), with the ability for the User to earn a certificate of completion for the completed course.
  • Customer shall be entitled to select only one course pursuant to this Enrollment Form and may not change that selection once submitted.
  • If Customer desires access to additional courses, Customer shall be required to submit an additional Enrollment Form and pay the fees for the additional course.
  • Only EMA Members are entitled to discounted pricing.


BY CHECKING THE ENROLL BUTTON ABOVE, THE PERSON ACCEPTING THIS ENROLLMENT FORM ON BEHALF OF CUSTOMER IS REPRESENTING THAT THEY HAVE THE LEGAL AUTHORITY TO BIND CUSTOMER TO LEGAL AGREEMENTS AND TO ACCEPT THIS ENROLLMENT FORM ON BEHALF OF CUSTOMER, THAT CUSTOMER AGREES THAT THE ELECTRONIC ACCEPTANCE OF THIS ENROLLMENT FORM SHALL HAVE THE SAME LEGAL EFFECT AS A HAND SIGNATURE OF THIS AGREEMENT BY CUSTOMER AND THAT CUSTOMER IS ACCEPTING THE TERMS OF USE AND THIS ENROLLMENT FORM.
